simple ios iphone app - oDesk

I need a simple ios app, fast.



The app will have 8 fields. (2 name fields, 2 email address fields, date/time, 2 integer fields)



A submit button.

The user will be shown a confirmation page.



The data from the form will be sent to a web service.



I need the app done today. The webservice is already written, and I can share the api with you. Pretty much a single post with the data attributes.
