Modification of Website by blackboxes

Hi there, I have a website at www.giftin.sg. It's a 1 page website that captures content from a form into the database. I need the website to send an email confirmation out, with the fields that they filled up, in a HTML email format... (Budget: $30-$250 SGD, Jobs: Graphic Design, HTML, PHP, Website Design)
