Design an app for spring water customers - oDesk

We are looking for someone to design an app for customers to download for free and nominate how many bottles of water they would require and weather its urgent or how long before they need it.

A simple app, that would then forward information from fields by email to company to notify of delivery required.

Fields for customer entry would include

Account number (rembered for future use)

Contact Number

Name

No. Of Bottles required

When delivery is required (drop down list including urgent, next delivery, less than 2 days, less than 5 days)

And additional field for Other services required : (customer entry)



and then send.

App should be made available for iOS and Android mobile phones.



Advice required for how to launch on google play and itunes is also required.
