Shopify - Newsletter Popup w/ MailChimp Integration - oDesk

I'm looking for help with creating a newsletter signup window for my Shopify store. I'm looking for the following:



1. Something that functions exactly the same at this one: http://www.nastygal.com/

2. Instructions on how to integrate the code into my Shopify store

3. A signup box that links with my MailChimp list. The box design (css) does not need to be customizable at this time; I will provide a background image.
